Submit a referral online or call our team. We’ll confirm eligibility and discuss your goals and funding options.

Your therapist visits you at home for a 60-minute assessment. Together you’ll set goals and design a tailored therapy plan.

Regular 45–60 minute home visits. Your therapist tracks progress and adjusts the approach over time to keep sessions meaningful.
