What is Enhanced Primary Care?

The EPC (Enhanced Primary Care) program, also known as Chronic Disease Management (CDM) program, is a Medicare-funded initiative in Australia that aims to improve the management and care of patients with chronic or complex health conditions in the primary care setting. The EPC program provides eligible patients with access to physiotherapy and exercise physiology services to better manage their chronic conditions and improve their overall health outcomes.

Under the EPC program, patients who have chronic or complex health conditions, such as diabetes, arthritis, chronic pain, or respiratory conditions, may be eligible to receive up to five subsidised allied health services per calendar year. These services are provided by qualified allied health professionals, such as physiotherapists and exercise physiologists.

Eligibility for the EPC program is determined by the patient’s GP, and patients are encouraged to discuss their eligibility and potential participation in the program with their GP.

Medicare rebates: Patients who receive allied health services under the EPC program may be eligible for Medicare rebates, which can help offset the cost of the services. The allied health professionals bill Medicare directly for the services provided, and the patient pays a gap fee depending on the allied health service and their individual circumstances.

The biggest benefit of assessing you at home is we get to understand your home environment.  As a result, we can make instant changes to anything that could be contributing to your aches and pains, or restrictions in movement.

Our initial consultations run for 1-hour and follow-up appointments for 30-45 minutes, potentially reducing the number of visits you’ll need and providing value for money.
