What is a Chronic Disease Management (CDM) Plan?
A Chronic Disease Management Plan (CDMP, now streamlined to GP Chronic Condition Management Plan or CCMP in Australia from July 2025) is a personalized health strategy developed with your GP for long-term conditions like diabetes, asthma, or heart disease, allowing access to Medicare-subsidised allied health services (physiotherapy, occupational therapy, dietetics, exercise physiology) and coordinating team-based care, often involving lifestyle changes and regular monitoring for better management

What Support Can Be Included
