  • Support Coordinators, plan managers or equivalent for NDIS and Home Care Packages providers

  • Mailing houses and other such providers
  • Credit, fraud and other security check providers
  • Marketing businesses engaged by us

You may request access to any personal information about you that we may hold. If at any time you believe that the personal information we hold about you is incorrect, incomplete or inaccurate, then you may request to change it.

We may need to verify your identity before providing you with your personal information. In some cases we may be unable to provide you with access to all of your personal information and where this occurs, we will explain why.

Hub and Spoke Health will destroy or de-identify your personal information as soon as practicable once it is no longer needed for the purpose it was collected. We may be required by law to retain your personal information after your relationship with us ends. In this case your personal information will continue to be protected in accordance with this Privacy Policy.
