Report-Writing
Report-writing is often a stressful time for both participants and support coordinators.
Hub and Spoke Health has a structured, evidence-based and detailed report writing process, to ensure that participants get the best chance of securing or keeping funding within their plan. We often collaborate with all stakeholders in the participant’s team, to ensure everyone is on the same page, and striving towards the same goals.
Initial Assessment
The initial assessment may occur over 1-2 sessions with the participant, so we can gain a full picture of their abilities, needs and goals.
We will always perform some treatment, which may consist of exercise prescription, manual therapy, education or home changes at the first assessment.
Scheduling
Setting a schedule for participants to be seen can be a complicated process.
Hub and Spoke Health strives to provide a schedule that is logistically sound, and takes into consideration the needs and wants of participants and their families.

Our initial consultations run for 1-hour and follow-up appointments for 30-45 minutes, potentially reducing the number of visits you’ll need and providing value for money.
