Telehealth Consultations Melbourne
The Australian Physiotherapy Association (APA) promotes Telehealth in their Choose Physio for Telehealth National Campaign. Telehealth allows us to see you on a screen in your home. It can be a face-time call, or a Skype-call, or in our case it’s a program called Physitrack that allows us to do exercises with you in real time, whilst giving you feedback along the way. If it sounds daunting…don’t worry. We’ll help to walk you through every step of the process.

Mobile Allied Health Assistants
What is a therapy assistant?
Telehealth Consultations offer a convenient and affordable way for patients to connect with clinicians for advice and care. With just a phone or computer, you can access the support you need to stay healthy and get the most out of our treatment plan.
If you need physio or exercise physiologist, and have the benefits, why not use them? There is a plethora of evidence showing Telehealth is as useful as in-person physio, for a variety of conditions. Not all, but some.
To get a referral for Telehealth for Medicare purposes, you need a GP visit which can now also occur via Telehealth. You can use GP2U to have a consult with your GP or call your local GP to see if they can do a Telehealth Mobile consult. Ask specifically for a Telehealth plan to see your physio. It’s similar to an EPC referral if you’ve had one before.
Private health insurance visits do not require you to get a Medicare referral.

NDIS & Support at Home
Navigating funding systems like the NDIS or Support at Home can be overwhelming — but we’re here to help.
Hub & Spoke Health is a registered provider under both programs. We understand the reporting, compliance, and communication these schemes require — and we’ve supported hundreds of participants and older adults to get the care they need at home.
We work closely with support coordinators, plan managers, care advisors and families to ensure services are goal-aligned, compliant, and tailored to each person’s needs.
Whether it’s physiotherapy, art therapy, dietetics or beyond — we help make the process simple.

Our mobile Allied Health services
Mobile healthcare allows you to receive tailored allied health support at home, rather than travelling to a provider. Across Australia, you can schedule a visit and receive care in the environment that matters most, your everyday space. When you book a mobile appointment with Hub & Spoke, our practitioner delivers personalised care designed around your needs, your goals, and your home set-up.
Our Hub & Spoke practitioners will bring their equipment with them so you get the full benefit of a clinic visit without leaving your home. The biggest benefit of assessing you at home is we get to understand your home environment. As a result, we can make instant changes to anything that could be contributing to your aches and pains, or restrictions in movement.
Our initial consultations run for 1-hour and follow-up appointments for 30-45 minutes, potentially reducing the number of visits you’ll need and providing value for money.
We’ll bring our experience and equipment to your home to provide the best evidence-based physiotherapy to support you in moving and living better.
We deeply understand fitness, exercise prescription, dosage, anatomy, physiology and most importantly, we prevent you from re-injuring yourself.
Art Therapy uses the creative process of art to enhance the emotional and physical well-being of children and adults. It can be delivered in the comfort of your own home, and we bring all art supplies with us.
Music therapy is a clinical and evidence-based use of music interventions to address the physical, emotional, cognitive, and social needs of individuals. It can be delivered in the comfort of your own home. Our music therapists bring an array of instruments with them to cater to any therapeutic musical preferences.
Our skilled therapists can help you regain independence and reach your goals. Whether you’re suffering from a physical injury or mental health condition, we’re here to help.
There are times in life when you need a little more—someone who has additional training and experience to support you (or your patient), or they need post-hospital care. Whether that’s cancer care, paediatric physio, transition care program, hospital in the home or aged care physio.
