Transition Care Program
The Transition Care Program (TCP) is a specialised program that provides short-term care and support to older adults who are transitioning from hospital to home or other care arrangements. The TCP is designed to help individuals who require additional care and rehabilitation after a hospital stay but do not yet require long-term residential aged care.

These services are tailored to the specific needs and goals of each patient and are aimed at maximising their recovery and restoring their ability to perform daily activities.
Home Modifications and Equipment
TCP may assess and provide home modifications or assistive equipment to ensure a safe and supportive environment for patients when they return home.
What about exercise physiology?
The TCP team works closely with the patient, their family, and other healthcare providers to develop and implement a personalised care plan, and to coordinate care services to ensure continuity of care.
