Mobile Healthcare Support for Older adults
Transition Care Program
The Transition Care Program (TCP) is a specialised program that provides short-term care and support to older adults who are transitioning from hospital to home or other care arrangements. The TCP is designed to help individuals who require additional care and rehabilitation after a hospital stay but do not yet require long-term residential aged care.

What is the goal of Transition Care Program?
The main goal of the Transition Care Program is to support patients in their recovery and rehabilitation, while also facilitating their safe and smooth transition back to their community or home. The program is typically delivered by a multidisciplinary team of healthcare professionals, including registered nurses, physiotherapists, exercise physiologists, art therapists, music therapists and other specialists, who work collaboratively to provide comprehensive care.
