The Transition Care Program (TCP) is a specialised program that provides short-term care and support to older adults who are transitioning from hospital to home or other care arrangements. The TCP is designed to help individuals who require additional care and rehabilitation after a hospital stay but do not yet require long-term residential aged care.

Our initial consultations run for 1-hour and follow-up appointments for 30-45 minutes, potentially reducing the number of visits you’ll need and providing value for money.
