What Is The Restorative Care Pathway?
The Restorative Care Pathway is a short-term, intensive program in Australia’s Support at Home program, providing multidisciplinary allied health for up to 16 weeks to help older adults regain function, strength, and independence after illness or injury, aiming to delay the need for higher-level care through personalised goals, exercise, and support. It’s funded separately from Support at Home funding, offers intensive support to achieve specific goals, and is designed to boost recovery and prevent decline, not replace ongoing care.

Who The Restorative Care Pathway Is For
The Restorative Care Pathway is for older Australians (65+, or 50+ for Aboriginal/Torres Strait Islander people) who need short-term, intensive support to regain independence after a setback like an illness, injury, or fall, with goals like improving mobility, balance, or function to stay at home longer. It’s for motivated individuals who can actively participate in a recovery plan with allied health and nursing to prevent further decline, not for those needing end-of-life or permanent care.

What Support Can Be Included
Support is time-limited and goal-based, delivered by a multidisciplinary team. Depending on needs, a restorative care episode include:
- Allied health support (commonly Physiotherapy and Occupational Therapy, other disciplines as needed)
- Ongoing monitoring and progress reviews
- Assistive technology, equipment, and home modifications through the Assistive Technology & Home Modifications (AT-HM) scheme (when needed)
How the Restorative Care Pathway works
Assessment & eligibility
You’ll need an aged care assessment completed by an aged care assessor and meet the pathway’s suitability criteria.
Your goal
A restorative care partner works with you (and your registered supporter if required) and the multidisciplinary team to create an individualised goal plan.
Intensive short-term support (up to 16 weeks)
You receive targeted services that focus on restoring function and building capacity.
Review & next steps
At the end of the episode, you can request a Support Plan Review to check goal progress and determine whether ongoing supports are needed.

As a result, we can make instant changes to anything that could be contributing to your aches and pains, or restrictions in movement.
Our initial consultations run for 1-hour and follow-up appointments for 30-45 minutes, potentially reducing the number of visits you’ll need and providing value for money.
